Report an Accident

When to Complete the Accident/Incident Report

All accidents, injuries and near misses (no injury) must be reported to a supervisor as soon as they occur. When an employee has a work related accident, incident or near miss, the Supervisor must complete the following:

  • Ensuring the employee received first-aid or medical treatment if required 
  • Investigate the incident
  • Along with the employee, the Supervisor shall complete the Accident/Incident Form
  • Once complete, the Accident/Incident form must be forwarded to the Health and Safety Department within 24 hours. The Workplace Safety and Insurance Board levies a fine of $1000 for late reporting. This charge is billed back to individual departments.
  • If an employee is injured and requires medical attention the Supervisor along with the employee will complete the Accident/Incident form when practicable to do so. If it is not practical to complete the report together the Supervisor will complete the report to the best of their abilities and forward it to the Health and Safety Department for further investigation

When an Employee requires Medical Attention

  • If the employee is seeking medical attention they must ask the Health Care Provider to complete a WSIB Form 8. The Form 8 is the Health Care Provider's report of injury that is sent to the WSIB, and outlines any work related accommodations or restrictions that may be required. The employee must return the completed Form 8 to their Supervisor as soon as possible to ensure a successful early and safe return to work.
  • All Documents must be forwarded to the Health and Safety Department within 24 hours of the accident/incident by email at or by fax at 519-971-3671. Original copies can be sent afterward through the inter-university mail.
